When you create the /boot/grub/menu.lst and you have a /boot partition mounted
in /mnt/lfs/boot, you should not add "/boot" before "/lfskernel" for grub to
boot the kernel.

The line: 
kernel --no-mem-option /boot/lfskernel root=/dev/hda4

should be:
kernel --no-mem-option /lfskernel root=/dev/hda4
