a) The book would explain mknod and devices.txt once make_devices replaces
MAKEDEV. Actually this is the whole point: Teach people about where
devices come from rather than just have them execute a command that
creates a million of them that will hopefully include all the devices
you'll ever need.

b) make_devices is VERY easy to customize. All you have to do is remove
the comment marker from the line of the device you want to create. 

c) The default make_devices will ship with all the important devices
activated.
