Done for PDF output. Now all the external links are in italic. The internal
ones (i.e. generated via xref) are in normal font, but can be also in italic
if wanted.

For HTML we can use italic globally for all links (included the navigational,
TOC, internal and external links) or match the links inside a sect1 (internal
and external ones) via the CSS code, or use italic only for external links
hacking the ulink template to include the {i} tag.
