About the future format for LFS

We need to change some of the elements and replace them with new elements, such as the screen and userinput combo. We also need to change decreapted Docbook 4 elements, such as ulink and bookinfo to link and info, respcrety. Also, we can remove elements we don't need.
 
We need to change the header to this:
 

   %general-entities;
   
   %allent;
]>
 
x must be replased by the root element.
 
We need to add the LFS namespace to the root tag too. No changes to the .ent files are needed. We need to, however, update the .xsl files.
